Is Your Petpick one!

All dogs, cats, and ferrets in Florida are at extreme risk of being infected by heartworms, a deadly disease transmitted by a mosquito bite.

It is crucially important that your pet be given a monthly preventative for protection. If you do not currently have a heartworm preventative, please ask us for information.

Our office is committed to providing the best quality care possible, and, though it is health-related, it is also a business.  We ask that you notify us in a timely manner if you are unable to keep an appointment.  This provides us with the opportunity to give your appointment time to someone who urgently needs to be seen.

We charge a $125 cancellation fee for each appointment cancelled less than 24 hours before the appointment time.  Thank you for your attention and help with this matter.

We are pleased to welcome you to our practice and include you in the Veterinary Healing Arts circle of friends. It is important that your pet is current on all of his/her vaccinations except if vaccination poses a detrimental medical risk.

It also important that your pet be free of internal parasites such as worms and external parasites like fleas, ticks, and ear mites. We have many safe and effective resources for you to use to keep you, your pet, and our other clients free from infestation – just ask!

If we are not satisfied that your pet meets the necessary health conditions, we will test and treat your pet for all parasites; this is a public health issue and is for the protection of your pet as well as our other clients.

Please check the box below and type your name to acknowledge that you understand this requirement and that you accept financial responsibility for your pet.

I understand the above statements and I agree to them

I hereby authorize Veterinary Healing Arts Center to examine, prescribe for, and/or treat my pet(s) as deemed medically necessary.  I assume responsibility for all charges incurred in the care of my animal(s) and understand that these charges will be paid at the time of service.  A deposit may also be required for drop-off patients.

I understand and agree that Dr. Rimar and the staff of Veterinary Healing Arts will use their best efforts to treat my pet.  I recognize that medical results from treatment are not and cannot be guaranteed, and results can vary widely dependant upon a variety of factors such as the condition of my pet, my consistency to carry through with recommend therapies (herbal, physical, training, etc.) and other factors.  If I percieve a problem with my pet's care and medical results, I agree to discuss the matter with Dr. Rimar before providing information to third parties; and I agree to binding arbitration of any disputes should the need arise.  I agree that the process of arbitration and the results are confidential between Dr. Rimar and me.  Should the need arise for arbitration, Dr. Rimar and I will each select and arbitrator and those two arbitrators will select a third party to hear the dispute; all fees for the arbitration will be paid by the losing party.
